At RISE, our Community Services team is here to support individuals with disabilities as they build the skills to live safely and confidently—both at home and out in the community.




What We Provide
The Community Services Department is tasked with supporting individuals with developmental disabilities in developing the skills needed to be safe and self-sufficient both at home and in the community.
Offsite
Community Integration
The goal of Community Integration is to foster relationships between the individual and the larger community through engagement in community activities, such as:

Social clubs
-
Recreation activities
-
Volunteering
This service must be facilitated in the community, i.e. away from the individual’s home or RISE facility. Activities of daily living such as grocery shopping, haircuts, errands, etc. should not be completed through this service.
Individualized Skill Development
The goal of Individualized Skill Development (ISD) is skill advancement to promote independence and self-sufficiency both at home and in the community. Skill advancement activities can include:

Cooking
-
Shopping
-
Laundry
-
Budgeting
-
Paying bills
ISD is encouraged to be facilitated both at home and in the community when applicable.
This service is designed for staff to coach participants in learning how to do specific tasks. Staff will not perform tasks for the individual such as completing laundry for the individual. The individual must be present and engaged in the activities.
